New patients please print, read and fill out the following forms and bring them with you for your initial consultation and treatment.
Directions are also written at the end of these forms for your convenience.
Fees
- New Fees effective July 1st 2011:
- First Treatment including consultation (1.5 hours) ~ $125
- Follow-up Treatments (1 hour) ~ $75
- Front & Back Acupuncture Treatment (1.5 hours) ~ $95
- Herbal Consultation only (1/2 hour) ~ $65
- Cupping (1/2 hour) ~ $50
- Tuning Forks (1/2 hour) ~ $50
- Tuning Forks (1 hour) ~ $100
- Housecalls $175 for new clients & $125 thereafter for Ventura
Student discount: $10 off each treatment.
Friend Referral Discount
If you were referred by a client or friend you may receive $25 off your initial visit. This must be mentioned at the time of booking the appointment. Thank You.
Cancellation Policy
As a courtesy to other patients, please cancel 24 hours in advance to avoid charges. Thank you for understanding.
